Send your complaint or feedback to the faculty. You will receive a ticket number and a tracking code to follow it up.

This form is for general complaints. Reports of sexual violence or harassment are handled by the PPKS task force through a separate confidential channel and must not be sent here.

Describe what happened, when, and where, as clearly as you can.
